In the debut film of Andrey Iskanov (director of Visions of Suffering and philosophy of a knife), a hitman who suffers from migraine headaches and traumatic flashbacks turns to self-trepanation to relieve his suffering. With each nail that is driven into his brain, he is able to perceive new and terrifying layers of reality.
